新柏氏膜式超薄细胞检测配合阴道镜检查对子宫颈病变的诊断

目的：评价新柏氏膜式超薄细胞检测配合阴道镜检查对子宫颈病变的诊断价值。方法：对2000例子宫颈疾病患者行新柏氏膜式超薄细胞检查初筛，细胞学专职人员进行最后诊断。对73例阳性病例行阴道镜下取活组织病理检查。结果：阳性涂片248例，占12．4％。其中子宫颈鳞癌4例；子宫内膜癌1例；子宫颈低度鳞状上皮内病变40例（占2．0％），其中人乳头瘤病毒感染占77．5％。该方法检查的敏感性为98．9％，特异性为90．9％，与阴道镜检查符合率为83．6％。结论：采用新柏氏膜式超薄细胞检测初筛，阳性病例配合阴道镜病理做最后诊断，能及早发现癌前病变。

Evaluation of thinprep pap test with colposcopy for the diagnosis of cervical diseases

Objective To assess the value of thinprep pap test with colposcopy for the diagnosis of early cervical diseases.Methods Two thousand cervical pap smears were done in our hospital from January 2004 to January 2005.The thinprep pap test smears were analyzed first,and final reports were made by cytologist.Cervical biopsies under the guidance of colposcopy were taken in 73 cases of abnormal findings.Results Two hundred and forty-eight(12.4%)were found abnormal pap smears out of 2000 cases,in which 4 cases were cervical squamous cancer and 1 endometrial adenocarcinoma.The low-grade squamous intraepithelial lesion and human papilloma virus infection most frequently happened in the younger patients.There were 40 cases(2.0%)of low-grade squamous intraepithelial lesion,among which human papilloma virus infection accounted for 77.5%.Thinprep pap test was used for primary selection,with the sensitivity of 98.9%,and the specificity of 90.9%.Conclusion Sexually transmitted diseases is closely related to early cervical disease,thinprep pap test should be done in the young women,especially high-risk patients.The positive cases of thinprep pap test must be examined with colposcopy and pathological biopsy.
