“三部”理筋手法治疗椎间盘源性下腰痛145例

目的：观察俯卧位-侧卧位-仰卧位＂三部＂理筋手法对椎间盘源性下腰痛（dicogenic low backpain,DLBP）患者的治疗效果。方法：将290例DLBP患者随机分为手法组和药物组,每组各145例,手法组进行俯卧位-侧卧位-仰卧位＂三部＂理筋手法治疗,药物组口服非甾体消炎止痛药,采用视觉模拟评分法、主观满意度分级及下腰痛评分法（low back outcome scone,LBOS）,分别在治疗前、治疗后近期（3周）、治疗后远期（1年）对2组患者进行评价。结果：手法组、药物组患者治疗后近期、远期VAS评分均显著降低（P〈O.O5）,远期VAS评分临床症状缓解率、患者主观满意度优良率及显效率手法组明显优于药物组（P〈0.05）。结论：俯卧位-侧卧位-仰卧位＂三部＂理筋手法治疗DLBP临床疗效显著。

Maneuver in Treating 145 Cases of Discogenic Low Back Pain

Objective： To observe the effects of＂Three postures＂ （prone-lateral-supine position） tendon-soothing maneuver in treating the patients with discogenic low back pain（DLBP）. Method：290 cases of patients with DLBP were randomly divided into maneuver group and medicine group on average. Maneuver group were administered by ＂Three postures＂ tendon-soothing maneuvers,medicine group were orally given by non-steroid anti-inflammatory drugs （NSAIDS）. The effects of both groups before the treatment, after the treatment for 3 weeks and 1 year were assessed by visual analogue scales （VAS）, degrees of subjective satisfaction and low back outcome scone（LBOS） score. Result： VAS of both groups after the treatment for 3 weeks and 1 year were decreased significantly （P〈0.05）. Symptomatic remission rate of VAS after treatment for 1 year, excellent rate of subjective satisfaction and effective rate in maneuver group were superior to that in medicine group （P〈 0.05）. Conclusion： ＂Three postures＂（prone-lateral-supine position） tendon-soothing maneuver is effective to treat the patients with discogenic low back pain.
