乌司他丁加氨甲苯酸对血液的保护研究

摘    要:
目的 观察乌司他丁加氨甲苯酸和抑肽酶对体外循环(CPB)的血液保护作用。方法 选择60例风湿性心脏病瓣膜置换术的患者,随机分为两组。乌司他丁加氨甲苯酸组(30例):乌司他丁10000U/kg,氨甲苯酸40mg/kg,于麻醉诱导后至CPB开始前缓慢静注半量,另半量加入预充液中随转机进入体内;抑肽酶组(30例):抑肽酶500万U,于麻醉诱导后至CPB开始前缓慢静注半量,另半量加入预充液中随转机进入体内。结果 两组输血量和术后24h胸液量无统计学差异。抑肽酶组偶2例发生过敏。结 论乌司他丁加氨甲苯酸和抑肽酶均有维持纤溶活性稳定、保护血小板功能,从而减少术中及术后出血。

Abstract:
Objective To observe the protection of ulinastatin and aminomethylbenzoic acid and aprotinin on blood fibrinolytic system during cardiopulmonary bypass.Methods Sixty patients were divided into three groups:ulinastatin and aminomethylbenzoic acid group(n=30):patients received ulinastatin 5000IU/kg and aminomethylbenzoic acid 20mg/kg iv after induction of anesthesia and another 5000IU/kg and 20mg/kg was added into the priming solution;aprotinin group(n=30):patients received aprotinin 250KIU iv after induction of anesthesia and another 250KIU was added into the priming solution.Results There was no significant decrese in 24-hour chest tube drainage between ulinastatin and aminomethylbenzoic acid and aprotinin groups.Conclusion Ulinastatin and aminomethylbenzoic acid and aprotinin are effective in stabilizing fibrinolytic system and preserving platelet function during CPB,leading to less postoperative blood loss.
