超声造影诊断不典型脂肪肝的价值

摘    要:目的探讨二维超声、彩色多普勒血流显像(CDFI)及超声造影在不典型脂肪肝中的应用价值。方法对31例脂肪肝伴肝内异常回声者行二维超声、CDFI及超声造影检查并与增强CT检查结果对照,其中8例在超声引导下经皮肝穿刺活检。结果穿刺活检显示,8例低回声结节中1例为肝细胞肝癌(HCC),7例为相对正常的肝细胞,周围高回声区均为肝细胞脂肪变性:二维超声误诊2例HCC,2例小的肝囊肿和1例肝血管瘤;CDFI误诊2例HCC;增强cT漏诊1例9mm×9mmHCC;超声造影无漏诊、误诊。结论超声造影在二维图像的基础上可便捷诊断不典型脂肪肝残存的“肝岛”及其病变性质。

Abstract:Objective To investigate the value of contrast-enhanced ultrasound in the diagnosis of untypical fatter fiver. Methods Thirty-one cases of fatty fiver with untypical hypoechogenic nodules underwent 2-dimensional ultrasound , CDFI and contrast-enhanced ultrasound examinations. The results were compared with those of contrast-enhanced CT in all cases and with pathological results in 8 cases . Results The pathological results showed hepatocellular carcinoma (HCC) in 1 case and normal hepatic cells with fatty change in 7 cases. Compared with pathology. 2 HCC, 2 small liver cysts, and 1 hemangioma were misdiagnosed by 2-dimensional ultrasound, 2 HCC were misdiagnosed by CDFI, 1 HCC was missed by contrast-enhanced CT, and no lesion was misdiagnosed by contrast enhanced ultrasound. Conclusions On the basis of 2-dimensional ultrasound, contrast-enhanced ultrasound is valuable in the differentia/diagnosis of the untypical fatty fiver diseases.
