肥胖少年的臂-踝脉搏波速度与心血管疾病危险因素的关系探讨

摘    要:
目的 探讨肥胖少年的臂-踝脉搏波速度(brachial-ankle pulse wave velocity, BaPWV),与诱发心血管疾病发生的血生化指标的相关性。方法 在北京市海淀区选取165例14~17岁的肥胖少年,男90例,女75例,测量体重、身高、血压后,计算BMI,采用动脉硬化检测仪测量臂-踝脉搏波速度(BaPWV),同时检测空腹血生化指标。分析BaPWV与肥胖相关指标,以及心血管疾病危险因素中血生化指标的关系。结果 过度肥胖组的BaPWV,与总胆固醇(total cholesterol, TC)、低密度胆固醇(low density lipoprotein cholesterol, LDL-C)、三酰甘油(triglyceride, TG)呈正相关关系;与体重、体质指数(body mass index, BMI)、收缩压呈正相关关系。按照BaPWV测量值分组,BaPWV增高组(高于正常范围,>P95)的BaPWV,与总胆固醇(TC)、低密度胆固醇(LDL-C)呈正相关关系;与年龄、体重、体重指数(BMI)呈正相关关系。BaPWV增高与暴露于过度...

Abstract:
Objective To investigate the relationship between brachial-ankle pulse wave velocity(BaPWV)and cardiovascular risk factors,especially blood biochemical indicators,in obese adolescents.Methods A total of 165 obese adolescents(90 males and 75 females),aged 14-17 years,were selected from Haidian District,Beijing for the study.The body weight,height and blood pressure,body mass index(BMI),BaPWV using atherosclerosis detectors were measured in all patients.And fasting blood samples were collected for measurement of biochemical parameters.The relationship between BaPWV and obesity-related indicators,as well as blood biochemical indicators of cardiovascular risk factors,was then analyzed.Results BaPWV of patients with BMI>P50 was positively correlated with total cholesterol levels,low-density cholesterol levels,and triglycerides levels(r=0.265,P=0.016;r=0.246,P=0.025;r=0.240,P=0.029);and with body weight,body mass index,and systolic blood pressure(r=0.275,P=0.012;r=0.304,P=0.005;r=0.268,P=0.014).When BaPWV was above the normal range(>P95),it was positively correlated with total cholesterol levels and low-density cholesterol levels(r=0.324,P=0.048;r=0.417,P=0.010).BaPWV levels elevated with increasing in age,weight,or body mass index(r=0.416,P=0.011;r=0.299,P=0.072;r=0.355,P=0.031).The ratio of association strength between BaPWV above the normal range(>P95)and excessive obesity of BMI>P50 was OR=2.159,with a 95%confidence interval of 1.011-4.613.The result suggested that excessive obesity of BMI>P50 could cause 2.159-fold greater risk of BaPWV above the normal range.Conclusion Among the blood biochemical indicators of cardiovascular risk factors,total cholesterol levels and low-density cholesterol levels,are positively correlated with excessive obesity(BMI>P50),or BaPWV above the normal range.For adolescents aged 14-17 years,excessive obesity(BMI>P50)can increase the risk of BaPWV above the normal range.
