MRI评价HIFU治疗子宫肌瘤的疗效

目的探讨高强度超声聚焦刀(HIFU)治疗子宫肌瘤的疗效和MRI对其的评价。方法确诊子宫肌瘤患者40例,共48个病灶,行HIFU消融治疗,治疗前后行MRI平扫及增强扫描认真比较肿瘤的信号变化、血供情况及体积。结果 HIFU治疗1个月后,T1WI上瘤体信号值平均增加32.13(t=-5.156,P<0.01),T2WI上瘤体信号值平均增加了4.73(t=-0.632,P>0.05)。MRI显示42个肿瘤内部无强化,边缘轻度环状强化,肿瘤血供完全阻断,其余6个瘤体内少许强化,再次HIFU治疗后肿瘤内部无强化,肿瘤血供完全阻断。瘤体体积治疗前(50.3±22.7)cm3,经HIFU治疗6个月后为(18.8±12.5)cm3,t检验显示治疗前后肿瘤体积变化差异有统计学意义(P<0.05)。结论 HIFU是治疗子宫肌瘤的一种安全、有效方法,MRI是其疗效评价的一种较好影像方法。

Effect of High Ultrasound Knife in Treatment of Uterus Myoma

Objective To study the effect of high ultrasound knife（ HIFU ） in treatment of uterus myoma and MRI to its appraisal. Method 40 cases including 48 focuses of infection were observed by MRI before and after the HIFU treatment. The results including the signal change, the blood supplies and size were compared with each other, gesu/ts After HIFU, T1WI myometrium signal on the value increased by 32. 13 on average （t=-5.156, P（0.01）; T2WI myometrium signal on the value increased by 4.73 on average （t=-0.632, P〉0.05）. After HIFU treats 1 month, MRI showed that 42 tumors corn pletely no enhancement, the tumor blood for block completely, while the tumors appeared a little contrast- enhanced at the periphery in 6 cases, but after HIFU treatment added, the blood supply of the tumor was completely blocked. After HIFU treatment, The average volume of myoma before the treatment was （50.3 ± 22. 7） cm3 and it was reduced to （18.8± 12.5） cm3 at 6 months after the treatment, the difference was statistically significant （P〈0.05）. Conclusion HIFU treatment fibroid is one kind of security,valid method of treatment and MRI was useful for the evaluation.
