中枢神经系统血管外皮细胞瘤的磁共振成像表现回顾性分析

目的总结中枢神经系统血管外皮细胞瘤的磁共振成像(MRI)表现。方法回顾性分析经手术后病理证实的9例中枢神经系统血管外皮细胞瘤的MRI影像资料。结果 7例位于颅内脑外,1例位于侧脑室三角区内,1例位于髓外硬膜内。MRI上,4例(44.4%)呈类圆形或椭圆形结节、团块,5例(55.6%)呈分叶状团块。T1WI上,5例(55.6%)呈与脑白质等信号的稍短T1信号,3例(33.3%)呈与脑灰质等信号的等T1信号,1例与灰质对比呈稍长T1信号。T2WI上,8例(88.9%)呈与脑灰质等信号的T2信号,1例呈稍长T2信号;6例(66.7%)伴囊变;8例(88.9%)与脑膜或脊膜相连,5例(55.6%)以宽基底相连,3例(33.3%)以窄基底相连;5例(55.6%)伴流空血管影;增强后9例均有不均匀明显强化,囊变坏死区无强化,4例(44.4%)有典型脑膜尾征;7例(77.8%)颅内病灶伴轻度水肿,2例伴骨质破坏。9例患者最终均通过病理确诊。结论中枢神经系统血管外皮细胞瘤的MRI表现具有一定特征性,最终诊断需依靠病理。

Magnetic resonance imaging features of central nervous system hemangiopericytoma

Objective To summarize the magnetic resonance imaging(MRI)features of central nervous system(CNS)hemangiopericytoma(HPC).Methods MRI data of 9 cases with HPC proved by pathology were analyzed retrospectively.Results Seven cases were located inside the skull and outside the brain,1 case was located lateral ventricles trigone and 1 case was located inside hard mask and outside spinal cord.The results of MRI showed the shapes of 4 cases(44.4%)showed round or oval,5 cases(55.6%)showed sublobe.On T1WI,5 cases(55.6%)presented shorter T1 equal signal compared with alba,3 cases(33.3%)presented T1 equal signal compared with gray matter,1 case presented slightly longer T1 signal compared with gray matter.On T2WI,8 cases(88.9%)presented equal T2 signal compared with gray matter,1 case presented slightly longer T2 signal compared with gray matter.Six cases(66.7%)presented necrosis,8 cases(88.9%)connected meninges.5 cases(55.6%)connected wide base and 3 cases(33.3%)connected narrow base;5 cases(55.6%)had the flow of air blood film.All the 9 cases underwent enhancement scanning presented obvious enhancement in the artery phase without necrosis area enhancement.Four cases(44.4%)had typical dural tail sign.Seven cases(77.8%)had intracalvarium focus and slightly edema,2 cases presented bone erosion.The 9 patients were diagnosed by pathology finally.Conclusion CNS HPC′s MRI has some characteristics and its final diagnosis depends on pathology.
