胃癌全胃切除调节型双通道空肠代胃重建术的临床应用

目的：观察胃癌全胃切除调节型双通道空肠代胃术的临床应用效果。方法：总结分析38例胃癌全胃切除调节型双通道空肠代胃术病人术中手术操作要点，观察重建手术花费时间、术后并发症发生情况。结果：全胃切除调节型双通道空肠代胃术平均耗时50．7min；术后并发症6例，发生率15．7％（6／38）。对照组Roux-en-Y重建代胃术47例，重建手术平均耗时47．9min；随访45例，术后并发症发生14例，发生率31．1％（14／45）。两组并发症比较P〈O．05。结论：胃癌全胃切除调节型双通道空肠代胃术式是安全可靠、效果满意的手术方式。

Clinical application of total gastrectomy with regulated double-tract jejunal interposition reconstruction in gastric cancer

Objective ：To investigate the effect of total gastrectomy with regulated double-tract jejunal interposition reconstruction in gastric cancer.Methods：To summarize and analyze the main points of the operation, time of reconstruction surgery and incidence of post- operative complications in 38 cases of total gastrectomy of gastric cancer with regulated double-tract gastric jejunal interposition recon- struction.Results：The average operation time was 50.7min.The postoperative complications occurred in 6 cases, accounting for 15,7%（6/ 38）.There were 47 cases of Roux-en-Y gastric reconstruction in the control group, with the average operation time of 47.9 min.Among followed -up 45 cases, postoperative complications occurred in 14 cases with the incidence rate of 31,3%（14/45）.Comparing complications between the two groups, the difference was significant （P〈0.05）.Concluslon：Total gastrectomy with regulated double-tract jejunal interpo- sition reconstruction in gastric cancer is a safe and reliable operation mode with satisfactory effect.
