91例胃肠道间质瘤的临床病理特征分析

目的探讨胃肠道间质瘤的临床病理学特征。方法收集2010年1月至2013年12月间91例经病理诊断的胃肠道间质瘤,对其临床病理资料进行回顾性分析,并对标本进行CD117、CD34、DOG1、Desmin及S-100检测。结果在91例胃肠道间质瘤患者中,有38例患者为恶性,21例为交界性,32例为良性。CD117呈阳性79例,占86.8%;CD34呈阳性69例,占75.8%;DOG1呈阳性74例,占81.3%;SMA呈阳性39例,占42.9%;Desmin呈阳性15例,占16.5%;S-100呈阳性4例,占4.4%。结论胃肠道间质瘤的发生部位多,形态学复杂,诊断还需要联合病理切片和免疫组化检测,CD117、CD34和DOG1三者联合检测可提高胃肠道间质瘤的诊断准确率,而SMA、Desmin及S100是重要的鉴别诊断指标。

A clinicopathological analysis of gastrointestinal stromal tumors in 91 cases

Objective To investigate the clinical and pathological features of gastrointestinal stromal tumors.Methods Totally 91 cases of gastrointestinal stromal tumors from January 2010 to January 2013 were retrospective analyzed.Expression of CD117,CD34,DOG1,Desmin and S-100 were determined by immunohistochemistry.Results 91 cases of patients were diagnosed as gastrointestinal stromal tumors,including 38 malignant,21 borderline and 32 benign.The expression of CD117 was positive in 79 of 91 patients with gastrointestinal stromal tumors（ 86.8%）,CD34 was positive in 69 of 91 patients with gastrointestinal stromal tumors（ 75.8%）,DOG1 was positive in 74 of 91 patients with gastrointestinal stromal tumors（ 81.3%）,SMA was positive in 39 of 91 patients with gastrointestinal stromal tumors（ 42.9%）,Desmin was positive in 15 of 91 patients with gastrointestinal stromal tumors（ 16.5%）,and S-100 was positive in 14 of 91 patients with gastrointestinal stromal tumors（ 4.4%）.Conclusion Gastrointestinal stromal tumors have more occurrence site and complex morphology,and diagnosis mainly combined with histology and immunohistochemistry.The combination of CD117,CD34 and DOG1 can can improve the diagnostic accuracy of gastrointestinal stromal tumors,and SMA,Desmin and S100 are important differential diagnostic indicators.
