骨周与关节改变对氟骨症诊断及分型分度意义的评价

摘    要:本文对18岁以上525例地方性氟骨症和41例正常对照组人群X线片的骨周和关节改变进行了分析。结果表明,骨质改变在先,骨周改变在后,20岁以下无骨周改变,20岁以上随着年龄的增大,骨周骨化阳性率及严重程度均不断增加。因此,对氟骨症诊断及分度的价值也相应提高。关节改变与骨周改变基本上呈平行关系,由于其特异性较差,故诊断价值次于骨周;硬化性骨周和关节改变的阳性率略低于疏松型和混合型,混合型中极度骨软化者骨周骨化不易显现。

Abstract:Radiographs obtained from 525 patients aged 18 years and over who had suffered from endemic skeletal fluorosis and from 41 normal subjects as controls were analysed for changes of peripheral osseous tissues and joints. It was found that the change of osseous substance preceded that of peripheral osseous tissues, that patients under 20 years of age had no peripheral osseous change, and that the incidence and severity of ossifications of peripheral bony tissues increased with the age for patients over 20 years, their value in the diagnosis, classification and differentiation of skeletal fluorosis increasing correspondingly. Arthrentasis was, on the whole, parallel to the peripheral osseous change, but its diagnostic value was inferior to that of peripheral osseous change owing to its poorer specificity. The sclerotic types of peripheral osseous change and arthrentasis had slightly lower frequencies than did the rarefied and mixed types. In respect of the mixed type, ossification of peripheral osseous tissues was not easily displayed in patients with the extreme from of osteomalacie.
