高血压社区管理效果分析

目的对高血压患者实施规范的社区管理，有效帮助高血压的治疗和减少并发症的发生。方法2007—2009年间对本社区2046例高血压患者实施社区管理，包括分层建档、健康教育、生活方式干预、药物治疗指导、分级管理和效果评估。结果实施社区管理后高血压患者对疾病知识知晓率、治疗率、控制率明显提高，而高血压并发心脑血管病的病死率明显下降。结论实施社区管理是控制高血压和心脑血管病的有效途径。

Analysis of the effect on community management of hypertension

Objective For the therapy of hypertension and deceanding of complication through normal community management of hypertension. Methods 2046 hypertension patients have been managed normally from 2007 to 2009, the way including of files,heath education, intervention of life style , guidance of drug treatment, management in grades and evaluation of effect . Results The rate of understanding , therapy and control has been increased significantly and the mortality of cerebrovascular disease and angiocardiopathy has been decreased through normal community management of hypertension. Conclusion Community management is an effective way of controlling hypertension ,cerebrovascular disease and angiocardiopathy.
