部分生化指标对酒精性脂肪肝与非酒精性脂肪肝鉴别诊断意义的探讨

目的探讨部分生化指标对酒精性脂肪肝与非酒精性脂肪肝鉴别诊断的意义。方法对57例酒精性脂肪肝及226例非酒精性脂肪肝患者的血清甘油三酯、总胆固醇、尿酸、血糖、清蛋白、总蛋白、丙氨酸氨基转移酶、天门冬氨酸氨基转移酶、天门冬氨酸氨摹转移酶／丙氨酸氨基转移酶、谷氨酰转移酶等生化指标进行测定及分析。结果酒精性脂肪肝组的天门冬氨酸氨基转移酶、天门冬氨酸氨基转移酶／丙氨酸氨基转移酶、谷氨酰转移酶、尿酸异常的百分数分别为73．7％、35．1％、49．1％、59．6％，非酒精性脂肪肝组分别为28．8％、12．4％、16．4％、31．4％，两组相比差异有统计学意义。结论天门冬氨酸氨基转移酶、天门冬氨酸氨基转移酶／丙氨酸氨基转移酶、谷氨酰转移酶、尿酸、甘油三酯、总胆固醇等生化指标的检测对酒精性脂肪肝与非酒精性脂肪肝的鉴别诊断有意义。

Alcoholic fatty liver and non - alcoholic fatty liver in patients with a comparative analysis of biochemical indicators

Objective To investigate the biochemical indicators characteristics of the alcoholic fatty liver and non - alcoholic fatty liver and to enhance the rate of clinical-differential diagnosis. Methods 283 cases of research subjects were divided into alcoholic fatty liver group and non - alcoholic fatty liver group, there were 57 cases in alcoholic fatty liver group and 226 cases in non - alcoholic fatty liver group ,testing and analyzing the patients of two groups with serum triglyceride, cholesterol, uricacid, fasting - piasmaglucose, albumin, totalprotein, alanine aminotransferase, aspartate histidineaminotransferase, as- partate histidine aminotransferase/alanine aminotransferase, glutamyltransferase and other biochemical indicators. Results In alcoholic fatty liver group, the abnormal rate of aspartate histidineaminotransferase, aspartate histidine aminotransferase/alanine aminotransferase, glutamyltransferase, uricacid was 73.7%, 35. 1% ,49. 1%, 59.6%. In non -alcoholic fatty liver group,the abnormal rate was 28.8%, 12.4%, 16.4% ,31.4%. The abnormal rate in alcoholic fatty liver group was higher than that of non - alcoholic fatty liver group. Conclusion Aspartate histidine aminotransferase, aspartate histidine aminotransferase/alanine aminotransferase, glutamyhransferase, uricacid and other indicators are to identify alcoholic fatty liver and non - alcoholic fatty liver effective diagnosis indicators.
