普伐他汀对扩张性心肌病心力衰竭患者心功能的影响

目的探讨普伐他汀对扩张性心肌病心力衰竭患者心功能的影响及影响机制。方法 36例扩张性心肌病心力衰竭患者随机分为对照组和治疗组各18例,分别在常规治疗基础上给予安慰剂和普伐他汀治疗,比较2组治疗前、治疗12个月后左心室舒张末期内径、左心室射血分数、白细胞介素-6、肿瘤坏死因子-α、同型半胱氨酸及血浆脑钠肽水平。结果治疗组治疗后与治疗前及对照组比较,左心室射血分数增加(P<0.05),白细胞介素-6、肿瘤坏死因子-α、同型半胱氨酸及脑钠肽水平下降(P<0.05),左心室舒张末期内径减小(P<0.05)。结论普伐他汀可能通过降低患者白细胞介素-6、肿瘤坏死因子-α、同型半胱氨酸及血浆脑钠肽水平减轻心室重构,从而改善心功能。

Effects of pravastatin on the heart function in patients with dilated cardiomyopathy and heart failure

Objective To evaluate the effects of pravastatin on patients with dilated cardiomyopathy(DCM) and heart failure(HF) and its mechanism.Methods Thirty-six patients with DCM and HF were randomly divided into pravastatin treatment group and placebo control group,with 18 patients in each group.All patients were followed up for twelve months.The left ventricular end-diastolic dimension(LVEDD) and ejection fractions(EF) were measured on ultrasound,and the levels of serum tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),homocysteine(Hcy) and plasma brain natriuretic peptide(BNP) were detected and compared before and after treatment between two groups.Results After the treatment with pravastatin for 12 months,EF increased,the levels of serum TNF-α,IL-6,Hcy and plasma BNP decreased,and LVEDD decreased in pravastatin treatment group compared with before treatment and placebo control group(P<0.05).Conclusion Pravastatin can relieve ventricular remolding and improve heart function by lowering the levels of TNF-α,Hcy and plasma BNP.
