重庆市剖宫产现状及其影响因素分析

目的 分析助产机构剖宫产现状及其影响因素,并提出相关解决措施.方法 对重庆市2003～2006 年产科质量调查资料进行分析.结果 开展剖宫产术的助产机构逐年增加,剖宫产率呈逐年上升的趋势,不同等级和地域的助产机构的剖宫产率不一样;新生儿窒息和产后出血的发生率随剖宫产率的升高而降低(P＜0.01),但围产儿死亡率并没有随之降低,反而升高(P＜0.01).结论 剖宫产率的升高在一定程度上可以降低新生儿窒息的发生率,但过高并不能相应降低围产儿死亡率.因此,应通过多种干预措施来降低剖宫产率.

Analysis of present status and the influence factors of cesarean section in midwifery hospital

Objective To provide countermeasures by analysis of present status and the influence factors of cesarean section. Methods We studied the data of Obstetric Quality Investigation from 2003 to 2006 in Chongqing.Results The number of midwife- ry hospital with cesarean section had increased gradually year.There was a increase trend of cesarean section rate.The rate was dissimilarity in different level or in different area.The rate of cesarean section had the positive correlation with the rate of the neo- natal asphyxia,the rate of postpartum hemorrhage(P<0.01),but had the negative correlation with the perinatal mortality(P<0.01).Conclusion The increased cesarean section rate can reduce the rate of the neonatal asphyxia,but further rise of cesarean sec- tion rate does not result in the corresponding decrease of the perinatal mortality rate.Therefore,we should reduce the cesarean sec- tion rate by interference methods.
