~(99m)Tc-葡聚糖乳内淋巴显像及其临床评价

本文报告26例正常人和35例乳腺包块~(99m)Tc-葡聚糖乳内淋巴显像的结果。正常人绝大多数双侧淋巴结数和(或)显影浓淡不对称,并发现左右交叉引流和先天性单链各2例;乳癌乳内淋巴转移者双侧或单侧淋巴链不显影,或一侧淋巴链仅低位淋巴结显影而高位缺如。本法判断淋巴受累的灵敏度为100％,特异性为85.2％,准确性为88.6％,假阳性为14.8％,假阴性为零。

CLINICAL EVALUATION OF ~(99m)Tc-DEXTRAN INTERNAL MAMMARY LYMPHOSCINTIGRAPHY

~(99m)Tc-dextran internal mammary lym-phoscintigraphy (IML) was performed in26 nosmal control subjects and 35 cases ofbre ast mass. In the 26 normal control subjects,cross drainage between parasternal lymphat-ics and an unilateral chain of parasternalnodes were observed in two cases each; bilat-eral parasternal node chain was seen in theremaining cases, but the number, size andintensity of radioactivity uptake of the par-asternal nodes were asymmetric bilaterallyin most of the cases. Abnormal patterns ofIML in the patients with internal mammarynode metastasis included nonvisualized bilat-eral or unilateral lymphnode chain and vis-ualized ipsilateral diaphragmatic node withcompletely visualized contralateral lymphnodechain. In predicting neoplastic: involvementof internal mammary lymphnodes. sensitivity,specificity, accuracy, false positive and falsenegative of IML were 100%, 85.2%, 88.6%,14.8% and 0%, respectively.
