血清肿瘤坏死因子-α与糖尿病肾病的相关性

目的:研究肿瘤坏死因子-α(TNF-α)在2型糖尿病肾病不同时期的患者中的变化,探讨它们与糖尿病肾病的关系。方法:采用放射免疫分析法测定30例单纯糖尿病组、30例早期糖尿病肾病组、30例临床糖尿病肾病组、30例健康对照组的空腹TNF-α水平。结果:TNF-α在早期糖尿病肾病组及临床糖尿病肾病组中均明显高于单纯糖尿病组、健康对照组(P<0.05或P<0.01),且随病情进展逐渐升高,单纯糖尿病组与健康对照组比较差异无显著性(P>0.05)。相关及回归分析显示TNF-α与尿白蛋白排泄率(UAER)呈正相关,在逐步回归分析中最后进入方程。结论:TNF-α在糖尿病肾病的发生、发展中起到重要作用。

Serum Level of Tumor Necrosis Factor-α and its Meaning in Diabetic Nephropathy

Objective:To investigate the changes of tumor necrosis factor-α(TNF-α)in different stages of the patients with type 2 diabetic nephropathy(DN) and approach its relationship with DN.Methods: Serum TNF-α was measured by radioimmunoassay in 30 patients with diabetic patients with no DN,30 patients with incipient DN,30 patients with overt DN,and 30 control subjects.Results:The Serum level of TNF-α was significantly higher with the development of illness in DN patients than that in diabetic patients with no DN and normal persons(P<0.05 or P<0.01).Serum level of TNF-α in diabetic patients with no DN was not different from that in normal persons(P>0.05).Correlation analysis indicated that TNF-α was positively correlated to urinary albumin excretion rate(UAER).Stepwise regression analysis showed that TNF-α entered regression in the final phase.Conclusion:Serum TNF-α plays a very important role in the occurrence and development in DN.
