Analysis and Countermeasures of Failure of Clavicular Hook Plate Internal Fixation for Acromioclavicular Joint Dislocation

Objective:To explore the reasons and countermeasures for the failure of clavicular hook plate internal fixation for acromioclavicular joint dislocation.Methods:A retrospective analysis of 185 patients treated with clavicular hook plate from January 2017 to April 2019,of which 19(10.27%)patients had failed treatment,were summarized.Result:A total of 185 patients in this group were followed up for 3 to 27 months.A total of 19 failed cases were found.The causes of failure are as follows:There were 4 cases of iatrogenic errors,accounting for 21.1%;6 cases of decoupling,accounting for 31.5%;4 cases of screw loosening and steel plate bending,accounting for 21.I%;5 cases of internal fixation fracture,accounting for 26.3%.Acromion im pact,shoulder p ain,and limited shoulder joint movement due to internal fixation failure After the steel plate and internal fixation were all removed,the situation improved,all confirmed by radiography or shoulder arthroscopy.Conclusion:Failure after clavicular hook plate internal fixation can be prevented.Intraoperative anatomical reduction,correct placement of the clavicle hook,the manipulators technique and strength of pressing the plate,and proper and appropriate shoulder joint exercise alter operation can avoid surgical failure.
