18F-FDG符合线路/CT显像在非小细胞肺癌立体定向放疗中的应用

摘    要:目的探讨18F-脱氧葡萄糖符合线路/CT显像在非小细胞肺癌(NSCLC)立体定向放疗中的应用。方法53例NSCLC患者于放射治疗前行18F-FDG显像检查,进行FDG分期,并将FDG分期和CT分期结果进行比较分析。结果18F-FDG符合线路/CT显像使26例NSCLC患者分期改变,其中分期升级20例,分期降级6例。11例分期升级者PET检查发现了远处转移灶,其治疗方案由根治性治疗改为姑息性治疗;FDG显像较CT多发现39个转移淋巴结,并对其进行了立体定向放疗。FDG显像较CT多发现远处转移灶22个,对转移病灶进行了姑息性放疗。结论18FFDG符合线路/CT显像改变了49.06%(26/53)NSCLC患者的临床分期,影响了其治疗策略及放疗计划。

Abstract:Objective To investigate the apply of 18F-FDG coincidence/CT imaging in the stereotactic radiotherapy of non-small cell lung cancer (NSCLC). Methods Fifty-three patients with NSCLC had been examined by 18F-FDG coincidence/CT and staged by FDG imaging before radiotherapy. The results of 18F-FDG coincidence/CT examination were compared with those of CT. Results The staging was changed in 26 patients because of 18F-FDG coincidence/CT findings, with 20 patients being upstaged and downstaged in 6 patients. Because of distant metastasis detected by FDG imaging, 11 patients received palliative treatment. Compared with CT, 39 more lymph nodes and 22 distant metastasis were found in FDG imaging, then received stereotactic radiotherapy and palliative treatment respectively. Conclusion 18F-FDG coincidence/CT imaging, by changing clinical staging in 49.06%(26/53) NSCLC patients, has impact on treatment strategy and treatment planning of stereotactic radiotherapy in NSCLC patients.
