应用外固定器治疗手部骨折

目的：寻求手部小管状骨折，特别开放骨折及掌、指骨头部粉碎性骨折的更为简便有效的治疗方法。方法：自1998年10月～2004年6月，应用自行设计的手部外固定器，在电视X光机透视下，经皮固定手掌、指骨骨折112例，其中开放骨折21例，掌、指骨头部粉碎骨折41例。结果：随访92例，随访时间3～12个月。骨折愈合86例，不愈合1例，骨髓炎2例，畸形愈合1例。骨折愈合时间4～10周。其中粉碎性骨折及开放性骨折愈合时间6～10周。手功能恢复：按照TAM标准评价，优良率85．8%。结论：手部外固定器结构简单，方便实用。固定掌、指骨稳固，特别对开放骨折及掌、指骨头部粉碎性骨折是简便有效的治疗方法。

Treating Fracture of Hand with External Fixator

Objective: To find an easy and effective way to treat the small tubular bone frature, especially open fracture of metacarpus head and phalanx head. Methods: 112 cases of fracture of metacarpus and phalanx were treated with hand external fixator under C-arm X-ray machine from Oct. 1998 to Jua 2004. There were 21 cases of open fracture and 41 cases of comminute fracture of metacarpus head and phalanx head. Results: 92 cases were followed-up for 3-12 months. 86 cases were union ,1 case was ununion, 2 cases of osteomyelitis and 1 case abnormal union were occurred. Time of union was 4-10 weeks. And time of union was 6-10 weeks for open and comminute fracture. According to TAM criteria, the exallent rate of the recorery of hand function was 85. 8%. Conclusion:The hand external fixator was chac-terised by easy operation , simple structure and firmly fixation, so it s a easy and effective way to treat open fracture and fracture of metacarpus head and phalanx head.
