Sysmex XE-5000在血液疾病中光学血小板检测的应用

[目的]探讨Sysmex XE-5000全自动血细胞分析仪计数血液病光学血小板的临床应用价值。[方法]用电阻抗法、光学法和手工法同时计数血小板（PLT），以手工法结果为参考，分别与电阻抗法（PLT—I）和光学法（PLT—O）进行比较。[结果]1．Sysmex XE-5000全自动血细胞分析仪PLT—I法和PLT-O法计数血小板（低、中、高值）有较好的重复性，变异系数（CV）均〈4．0％。2．对于血小板减少的血液性疾病患者，PLT—O法计数血小板与显微镜法比较差异无统计学意义（P〉0．01），PLT—I法计数血小板与显微镜法比较差异有统计学意义（P〈0．01）；对于血小板增多的血液性疾病患者PLT—I法计数血小板与显微镜法比较差异无统计学意义（P〉0．01），PLT—I法计数血小板与显微镜法比较差异有统计学意义（P〈0．01）；[结论]Sysmex XE-5000血液分析仪是测定血液病患者血小板计数较理想仪器，光学法是解决异常血液标本检测血小板计数的一个重要手段，而对于血小板增多的患者应选用阻抗法。

CLINICAL EVALUATION OF OPTICAL PLATELET ASSAY BY SYSMEX XE -5000 HEMATOLOGICAL ANALYZER IN HEMATOIOGICAL DISEAS

[ Objective] To evaluate the value of optical platelet assay in hemopathy by Sysmex XE -5000 hematological analyzer. [ Methods] We counted the platelets with resistant method, optical method and microscopic method （reference method）, and analyzed the correlation between the results of resistant method and optical method. [ Results] 1. The precisions of PLT counts in low,normal and high concentration were good （coefficientof vailation CV 〈 3.0% ） by both PLT - I and PLT - O methods with Sysmex XE - 5000.2. In the patients with thrombocytopenia, the PLT count determined by the methods of microscopy and PLT - O showed no significant difference （ P 〉 0.01 ）, but there was significant difference between the method of PLT - I and microscopy （ P 〈 0.01 ）. In the patients with thromboocytosis, the PLT count determined by the methods of microscopy and PLT- I showed no significant difference （P 〉 0.01 ）, but there was significant difference between the method of PLT- O and microscopy（P 〈 0.01 ）. [ Conclusion] For patients with hematological disease, the Sysmex XE -5000 analyzer can satisfy needs of determining blood platelet count in the clinical laboratory. PLT - O is an accurate way to assay platelets of thrombocytopenia sample. But for thromboocytosis sample , the results should be checked by PLT - I method.
