孕妇血脂水平与新生儿出生体重关系的研究

摘    要:目的探讨孕期血脂水平的变化及其对新生儿出生体重的影响,为降低异常出生体重儿的发生率、合理控制孕期饮食提供依据。方法2006年1月至2008年12月,在厦门市妇幼保健院营养门诊随机抽取571例孕妇,分别于入选初始(孕20周前)、孕26~27周、孕35~36周留取空腹血清样本,检测血清胆固醇、三酰甘油水平,并追踪新生儿出生体重,分析不同孕期血脂变化及其对新生儿出生体重的影响。结果孕妇血脂(三酰甘油、总胆固醇)水平随孕期的进展逐渐升高(P〈0.001),孕中期血清总胆固醇(TC)水平随新生儿出生体重增加而上升,两者呈正相关(r=0.376,P〈0.05)。孕20周前及孕中期末,高血脂组孕妇的新生儿出生体重高于正常血脂组(P〈0.05)。新生儿出生体重越重,孕妇孕中期末及孕晚期的TC水平越高(P〈0.05)。结论孕期血脂水平呈逐渐增高的趋势,高血脂水平对新生儿出生体重会造成影响。孕中、晚期TC水平异常可能是导致新生儿出生体重异常的一个重要因素。

Abstract:Objective To explore the changes of triglyceride and cholesterol during the pregnancy, analyze the influence that the changes affect on the born weight of the infant, provide some suggestion for the pregnant women to control their dietary pattern, and low down the incidence rate of the abnormal born weight. Methods From Jan 2006 to Dec 2008, we selected 571 pregnant women randomly in Maternal and Child Health Hospital in Xiamen City. During the pregnancy, we collected the blood serum in different periods to test the level of the triglyceride and cholesterol. Finally, we also pursued the born weight of the in- fant to analyze the changes of the triglyceride and cholesterol during the pregnancy, and the influence that the changes affect on the born weight. Results The level of the triglyceride and cholesterol rose continually, and during the period of 26 th-27 th week, the level of the total cholesterol rose with the rising of the born weight, the correlation coefficient is 0. 376 (P(0.05). During the period of 20 th and 26 th-27 th week, if the pregnant women had the high level of triglyceride or cholesterol, their in- fant's born weight would be larger than that of the pregnant women who had the normal level of triglyceride and cholesterol (P〈 0.05). During the period of 26 th 27 th week and 34 th 36 th week, the level of cholesterol was higher if the born weight was larger (P〈0.05). Conclusion The level of the triglyceride and cholesterol has the tendency of rising. The high level of the two nutriments would do some influence on the born weight.
