骨肉瘤的治疗进展

骨肉瘤是儿童和青少年期最常见的恶性骨肿瘤,发生转移早,病死率高,其治疗一直都是世界难题。既往骨肉瘤仅限于手术治疗,5年生存率为20%左右。随着对肿瘤发生机制认识的不断深入,针对骨肉瘤的治疗已经取得了较大进展,从传统截肢手术到保肢手术,从新辅助化疗支持下的保肢综合治疗到分子水平的基因治疗,从骨肉瘤细胞端粒酶靶向骨肉瘤干细胞治疗等,均取得了跨越式的发展。特别是新辅助化疗下的保肢综合治疗在临床顺利、成功地开展,有效提高了骨肉瘤患者的5年生存率,并可使患者免受截肢的痛苦,保肢手术已成为肢体骨肉瘤外科治疗的标准。

Progress in Treatment of Osteosarcoma

Osteosarcoma is the most common malignant bone tumor in the period of children and adolescents with early metastasis and high fatality.Its treatment has always been a problem in the world.Previously osteosarcoma was limited to the operation treatment,and the 5 years survival rate was about 20%,Along with the continuous deepening understanding of neoplastic disease occurrence mechanism,the treatment of osteosarcoma has made great progress,from tradional amputation operation to salvage operation,from neoadjuvant chemotherapy supported limb salvage treatment to the molecular level gene therapy,from osteosarcoma cells telomerase targeted therapy to osteosarcoma stem cell therapy.Especially the successful application of neoadjuvant chemotherapy supported limb salvage comprehensive treatment in clinical,has effectively improved the 5 years survival rate,while keeping the patients from amputation pain,therefore,limb salvage operation has become the standard for surgical treatment of limb osteosarcoma.
