对比应用锁骨钩钢板或克氏针治疗锁骨远端骨折和肩锁关节脱位

目的探讨分别应用锁骨钩钢板或克氏针治疗锁骨远端骨折和肩锁关节脱位的优缺点。方法分析自2000年8月～2004年8月分别用两种方法治疗48例患者的疗效,其中锁骨钩钢板组22例平均随访1.5年,克氏针组26例平均随访2年,对比两组在手术难易程度,肩关节功能恢复、疼痛缓解,术后并发症(肩锁关节再脱位、内固定物松动移位、骨折延迟愈合和骨不连、伤口感染等)的发生率上两种方法的好坏。结果两种方法在手术难易程度方面无显著性差异(P>0.05);锁骨钩钢板组在肩关节功能恢复、疼痛缓解以及减少术后并发症发生方面均优于克氏针组(P<0.05)。结论锁骨钩钢板是一种优于克氏针治疗锁骨远端骨折和肩锁关节脱位的新方法。

A comparative study of clavicular hook plate and kirschner wire fixation in treating the distal clavicle fracture and the acromioclavicular joint dislocation

Objective To analyze the effect of clavicular hook plate (CHP) and kirschner wire(K-wire) fixation in treating the distal clavicle fracture and the acromioclavicular joint dislocation. Methods From Aug 2000 to Aug 2004, 48 case were treated by two kinds of methods respectively, CHP was used in 22 cases and mean follow-up was 1.5 years, K-wire was used in 26 cases and mean follow-up was 2 years, operation difficulty, shoulder pain symptoms and function, postoperative complications were assessed. Results Two groups had no significant difference in operation difficulty(P>0.05), the CHP group had better shoulder function and less shoulder pain symptoms and fewer postoperative complications than the K-wire group(P<0.05). Conclusions CHP is better than K-wire in treating the distal clavicle fracture and the acromioclavicular joint dislocation on pain abatement, postoperative complications reduce and the recovery of shoulder function.
