头颈部病变CT灌注参数与血管生成因子的相关性研究

摘    要:
目的探讨CT灌注参数与血管生成因子表达在头颈部病变中的临床应用价值及其相关性。方法本组46例头颈部病变行术前CT灌注检查,并使用软件计算感兴趣区的各灌注参数(BF、Bv、MTT、PS)值。术后对病变标本行免疫组化染色,检测其MVD计数和VEGF积分。统计分析头颈部良恶性肿瘤的MVD计数和VEGF积分差异的显著性,并分析各CT灌注参数值与MVD计数和VEGF积分的相关性。结果CT灌注参数值(BF、Bv、MTF、PS)在头颈部恶性肿瘤组和良性病变组差异存在统计学意义(P〈0.05),MVD计数与VEGF得分在2组间亦存在显著性差异(P〈0.05)。MVD计数与BF值和BV值呈正相关(r:0.563、0.355,P〈0.05),与MTY值呈负相关(r=-0.535,P〈0.05),与Ps值无相关性(r:0.250,P〉0.05),与VEGF得分呈正相关(r=0.697,P〈0.05)。结论头颈部病变cT灌注检查简便易行,其灌注参数对评估病变微血管状况有一定参考应用价值。

Correlation between CT perfuson parameters and angiogenesis factor in head and neck tumor

Abstract:
Objective To investigate the correlation between the CT perfusion parameters and expression of microvessel density(MVD) and vascular endothelial growth factor (VEGF) in head and neck lesions. Methods Forty-six cases of head and neck lesions were scanned by spiral CT to calculate the perfusion parameters of ROI. Histopathological slides were prepared for the anti-CD34 and VEGF immunohistochemical staining. The CT perfusion parameters were correlatively studied with MVD and VEGF. Results There was significant difference between benign and malignant lesions of head and neck with CT peffusion parameters. MVD and VEGF existed significant difference between malignant and benign le- sions (P 〈 0.05 ). BF and BV was positively correlated with MVD( r = 0. 563,0. 355, P 〈 O. 05 ) , MTT and MVD showed a negative correlation( r = -0. 535, P 〈 O. 05 ), but there was not correlative between PS and MVD( r = O. 250, P 〉 O. 05 ). MVD and VEGF were positively correlation ( r = O. 697, P 〈 O. 05 ). Conclusion CT perfusion parameters in head and neck lesions are correlated with MVD and VEGF. The CT perfusion could be helpful for differentiating benign from malig- nant tumors.
