2型糖尿病与非酒精性脂肪肝相关性研究

目的 探讨2型糖尿病与非酒精性脂肪性肝病（NAFLD）的相关性。方法 对656例确诊2型糖尿病患者行血糖、血脂、肝功能等生化指标检测和肝脏B超检查。结果 ①656例糖尿病患者B超证实脂肪肝430例,占65.54%;②分脂肪肝组（430例）,无脂肪肝组（226例）：脂肪肝组空腹血糖（FBG）、总胆固醇（TC）、甘油三酯（TG）、低密度脂蛋白胆固醇（LDL-C）、丙氨酸氨基转移酶（ALT）、谷氨酰转肽酶（GGT）、收缩压（SBP）、舒张压（DBP）,体重指数（BMI）、腰围/臀围比值（WHR）等均值明显升高（P〈0.05或P〈0.01）,HDL-C均值明显降低（P〈0.05）。结论 糖尿病合并脂肪肝患者存在明显脂代谢紊乱和中央型肥胖,血脂紊乱、肥胖、高血压是其独立危险因素。

A correlational study of type 2 diabetes mellitus with non-alcoholic fatty liver disease

Objective To study the correlation between the Type 2 diabetes mellitus with non-alcoholic fatty liver disease（ NAFLD）. Methods 656 cases of type 2 diabetes were enrolled in this study. Their blood sugar,blood fat and liver functions were tested,and a liver ultrasound was performed. Results ①430 of 656 cases of diabetes were with fatty liver by a liver ultrasound（ 65. 54%）; ②the levels of fasting blood glucose（ FBG）,total cholesterol（ TC）,triglyceride（ TG）, low density lipoprotein cholesterol（ LDL-C）,alanine aminotransferase（ ALT）,gamma glutamyl transpeptidase（ GGTP）, systolic blood pressure（ SBP）,diastolic blood pressure（ DBP）,body mass index（ BMI）,waist / hip ratio（ WHR） were higher in the patients with fatty liver than those without fatty liver（ P〈0. 05 or P〈0. 01）,instead,HDL-C was lower（ P〈0. 05）. Conclusion The patients with type 2 diabetes and fatty liver usually accompanied by obvious disorder of lipid metabolism and central obesity. The dyslipidemia,obesity and high blood pressure is the independent risk factors.
