外伤性腹膜后血肿195例临床分析

目的:通过对195例外伤性腹膜后血肿的临床分析以提高对外伤性腹膜后血肿的早期诊断和早期治疗。方法:回顾性分析我院1991年3月至2007年7月收治的195例外伤性腹膜后血肿的临床资料。男146例,女49例。致伤原因:撞击伤60例,挤压伤15例,踢伤20例,坠落伤43例,刺伤57例。损伤部位:腹部135例,腰背部5例,会阴3例,右大腿1例,下段脊柱骨折24例,骨盆骨折27例。单纯腹膜后血肿22例,合并其它发多伤173例。损伤的腹膜后内脏及主要大血管有腹膜后十二指肠7例、胰8例、腹膜后结肠20例、腹膜后上段直肠15例、肾9例、肾蒂1例、腹主动脉1例、肝后下腔静脉破裂1例、第二肝门以下下腔静脉侧壁破裂1例、胰后门静脉侧壁破裂1例。治疗方法:130例腹膜后血肿末做探查,手术探查腹膜后血肿65例。结果:治愈184例(94.3%),腹主动脉刺破死亡1例(0.5%),合并严重多发伤死亡10例(5.1%)。结论:早期诊断和早期治疗外伤性腹膜后血肿对降低死亡率,提高腹膜后损伤治愈率具有重要的临床意义。

Clinical Analysis on 195 Cases Traumatic Retroperitoneal Hematoma

Objective: To improve the early diagnosis and treatment for traumatic retroperitoneal hematoma.Method: We analyzed 195 cases datas of traumatic retroperitoneal hematoma from Mar.1991 to Jul.2007(male,146,female 49).The cause and injury part were exposed thoroughly.65 cases with retroperitoneal hematoma underuent exploratory operation of hematoma,the other 130 cases without.Result: The cured rate,death from abdorninal artery rupture,complicated with severe multiple complications was 94.3%(184 cases),0.5%(1 case) 5.1%(10 cases) respectively.Conclusion: There's significant value to cure traumatic retrogeritoneal and decrease the death rate with early diagnosis and treatment.
