息肉状脉络膜血管病变所致玻璃体积血的微创玻璃体切除术

目的探讨息肉状脉络膜血管病变所致的玻璃体积血微创玻璃体切除术治疗的临床效果。方法回顾性分析2012年1月至2013年12月以大量玻璃体积血收住我科治疗，术后经吲哚菁绿眼底造影确诊为息肉状脉络膜血管病变者21例（21只眼），观察术后的视力恢复及并发症情况。结果术后1个月随访时视力较术前提高者15只眼（71．4％）。与手术前相比，手术后视力≥0．05的患者所占比例有明显增高（9．5％到38．1％），两者的差异具有统计学意义（x^2=4．72，P〈090．05）。21只眼中有9只眼硅油填充，4只眼C3F8填充，8只眼无填充物。前房积血及其引起的高眼压是常见的术后并发症。结论微创玻璃体切除术治疗息肉状脉络膜血管病变所致的玻璃体积血效果肯定，可以使大部分患者视力提高，并且为下一步的联合治疗提供条件。

Micro-invasive vitrectomy for vitreous hemorrhage caused by polypoidal choroidal vasculopathy

Objective To investigate the clinical efficacy of 23G vitrectomy for vitreous hemorrhage caused by polypoidal ehoroidal vasculopathy（PCV）. Methods The clinical data of 21 eyes of 21 patients who were diagnosed as vitreous hemorrhage caused by PCV and underwent 23G vitrectomy between January 2012 and December 2013 in our hospital were retrospectively analyzed. The visual acuity （VA） and surgical complications were observed. Results The BCVA was improved in 15 eyes （71.4%） at one month after surgery. The proportion of patients whose VA better than 0.05 was 9.5% before surgery and 38.1% after surgery,and the difference was statistically significant（x^2 =4. 72 ,P 〈0.05）. Among 21 eyes, 9 eyes were given silicon oil tamponade, 4 eyes were given C3 F8 tamponade and 8 eyes with no tamponade. The most common complications were hyphema and hyphema-related high intraocular pressure （IOP）. Conclusion 23G vetrectomy is effective for the treatment of vitreous hemorrhage in PCV patient which can improve the patients＇ vision and provide a good condition for further treatment.
