PET／CT与 MRI对妇科恶性肿瘤的诊断价值及意义

目的：探讨正电子发射计算机断层显像（ PET/CT）与核磁共振成像（ MRI）在妇科恶性肿瘤中的诊断价值及意义。方法选取2010年2月至2013年2月在武警浙江省总队医院应用PET/CT 与MRI检查的妇科恶性肿瘤患者40例，比较分析两种检查对妇科恶性肿瘤的诊断价值。结果 PET/CT与MRI检查对原发病灶的检出率分别为87．50％和60．00％，差异无统计学意义（χ2＝0．27，P＞0．05）；40例患者中PET/CT对淋巴结转移病例诊断敏感性、准确率均高于MRI，差异具有统计学意（χ2值分别为4．59、8．66，均P＜0．05）；PET/CT与MRI共同检出转移的淋巴结共98个，与手术证实淋巴结数目107个相比，联合检出率为91．58％；经比较PET/CT检出率高于MRI，差异具有统计学意义（χ2＝43．19，P＜0．01）。结论 PET/CT检查是一种安全、有效、准确率、敏感性高的诊断方法，并且能准确的检出妇科恶性肿瘤转移的淋巴结数目，对临床妇科恶性肿瘤的诊治具有重要的应用价值。

Diagnostic value and significance of PET/CT and MRI for gynecologic malignant tumors

Objective To explore the significance of PET/CT and MRI in diagnosing gynecologic malignant tumors.Methods Forty patients with gynecological malignant tumors accepting PET/CT or MRI examination in Armed Police Corps Hospital of Zhejiang Province were selected from February 2010 to February 2013, and the diagnostic value of two methods on malignant tumors was compared.Results The detection rate of PET/CT and MRI for primary lesion was 87.50%and 60.00%, respectively, and there was no significant difference （χ2 =0.27, P〉0.05）.The sensitivity and accuracy of PET/CT in diagnosing lymph node metastasis cases were higher than those of MRI, and the differences were significant （χ2 value was 4.59 and 8.66, respectively, both P〈0.05）.Totally 98 metastatic lymph nodes were detected by the combination of PET/CT and MRI methods, and there were 107 confirmed by surgery.The detection rate of combined examination was 91.58%, which was higher than that of MRI with significant difference （χ2 =43.19, P〈0.01）.Conclusion PET/CT examination for gynecologic malignant tumors is safe, effective, accurate and sensitive.It can detect the accurate number of metastatic lymph nodes, which is very important to the diagnosis and treatment of gynecological malignant tumors in clinics.
