胃癌根治术前应用立止血对凝血功能变化的影响

目的:探讨胃癌根治手术(D2,D3)对患者凝血功能的影响以及术前应用立止血对凝血功能的影响.方法:40例胃癌根治手术患者随机分为立止血及对照二组.术前5 min分组静注立止血2 kU或生理盐水5 ml.分别于给药前、给药后30 min和1 h及术毕抽取静脉血,检测血浆凝血酶原时间(PT)、部分凝血活酶时间(APTT).结果:对照组给药后30 min、1 h及术毕,PT、APTT均延长,与术前相比差异有显著性(P＜0.05).立止血组术中PT、APTT延长,但与术前比较差异无显著性(P＞0.05);术毕PT、APTT延长,与对照组比较差异无显著性.结论:胃癌根治术前静注立止血2 ku可使术中凝血时间趋于正常,从而减少术中渗血,而对术后凝血时间影响不大.

Effect of preoperative application of Reptilase on hemoagglutination in patients with radical gastrectomy for gastric cancer

Objective:To evaluate the effect of radical gastrectomy(D2,D3)and preoperative application of Reptilase on hemoagglutination in patients with gastric cancer,respectively.Methods: Forty patients with gastric cancer undergone radical gastrectomy were randomly divided into Reptilase group and control group.The groups were respectively received intravenous injection of Reptilase 2ku or isotonic NaCl 5ml five minutes before operation.Venous blood was drawn-off to study the change of PT and APTT before administration,half an hour and one hour after administration,and after operation respectively.Results: The numerus of PT and APTT increased in the control group,including half an hour and one hour after administration and postoperative groups,which was significantly different from that in the preoperative group(P<0.05).The numerus of PT and APTT increased in the Reptilase group,including half an hour and one hour after administration groups,but this finding was not significantly different from that in the preoperative group(P>0.05).Although PT and APTT all increased after operation,there was no significant difference between Reptilase group and control group.Conclusion: Clotting time in radical gastrectomy for gastric cancer be normal,if the patients receive intravenous injection of Reptilase 2ku before operation.It will also reduce capillary hemorrhage and will not give an impact to clotting time after operation significantly.
