摘    要:目的:研究静脉应用胺碘酮治疗快速心律失常导致的急性肝损害及其转归.方法:观察静脉用胺碘酮治疗快速心律失常导致急性肝损害例的临床表现、肝脏生化指标的变化及其临床转归.结果:9例患者出现肝脏损坏时静脉胺碘酮累积量平均为1392.7mg,首日负荷量平均为182.6mg.用药后9例次日转氨酶即开始升高,d3达峰值,患者无临床症状,停药后保肝治疗,约八周转氨酶降至正常,无因肝损害死亡病例.结论:静脉应用胺碘酮可以引起急性肝损害,尽管少见但具有潜在的致命性,因此监测肝脏功能是有必要的.

Abstract:Objective: To study hepatotoxicity in tachycardia arrhythma with intraverous amiodarone.Method: To observe 9 cases characters of clinical and biochemistry with acute liver impaired induced by intravenous amiodarone.Result: Liver enzymes(ALT,AST) raised when average cumulative dose is 1392.7mg, only two paitents have symptoms.ALT and AST droped to normal level after intraverous amiodarone were stopped about 8 weeks in most patients.Conclusion: Intraverous amiodarone-related hepatotoxicity is rare,but it has fatal feature.Therefore,patients will be monitored to test liver enzme after intravenous amiodarone.
