应用关节镜下松解和几丁糖注射治疗膝关节粘连

摘    要:
为研究关节镜下粘连松解配合关节腔内注射几丁糖治疗膝关节粘连的可行性和疗效,运用关节镜行粘连松解术并向关节腔内注射几丁糖的方法治疗膝关节粘连15例(A组),与同期单纯关节镜下行粘连松解术治疗的12例(B组)及切开松解的11例(C组)进行比较.随访结果显示,术后1、3、6个月时,A组关节活动度均明显优于B、C两组(P<0.05).认为关节镜下松解术具有创伤小、恢复快、并发症很少等优点,是治疗膝关节粘连较为理想的方法,几丁糖注射可预防松解术后再粘连的发生.

Abstract:
The objective of the paper is to study the feasibility and therapeutic effect of arthroscopic lysis of adhesion (ALA) and intra-articular injection of Chitosan (IIC) for treating knee joint adhesion (KJA). Fifteen cases of KJA were treated by ALA and IIC (Group A) and compared with 12 cases treated by ALA alone (Group B) and 11 cases treated by open lysis (Group C). The results showed that Group A had an obvious advantage over Group B and Group C in the joint activity one, three and four months after operation (P < 0.05). It was believed that arthroscopic lysis has some advantages, such as little injury, rapid recovery, few complications and so on, being the rational therapy for KJA; Chitosan injection could prevent the adhesion recurrence after lysis.
