脑弥漫性轴索损伤临床表现及CT诊断的探讨

目的　探讨脑弥漫性轴索损伤的临床及CT表现。方法　分析 60例脑弥漫性轴索损伤的临床及CT资料。结果　本病的临床表现 :①头部受加速旋转性外伤 ;②伤后立即昏迷 ;③生命体征紊乱 ;④无明确神经定位体征。CT表现 :①大脑皮髓质交界处和 /或大脑中轴多发小出血灶 ;②弥漫性脑肿胀 ;③脑室、脑池受压变小或消失 ;④中线无移位或轻度移位 (<0 5cm) ;⑤合并其它颅脑损伤。结论　临床和CT表现二者结合 ,可对本病作出诊断

Clinical symptoms and CT signs of diffuse axonal injury

Objective To investigate the clinical findings and CT diagnositic standards of diffuse axonal injury (DAI). Methods The clinical symptoms and CT findings of 60 cases of DAI were analyzed. Results The clinical findings of DAI were as following: accelerant and spiral injury of head; immediate coma after injury; abnormalities of vital sign; no local neurological signs. The findings of CT were as following: multiple small hemorrhagic lesions at the cerebral cortical medullary junction or the axial area; diffuse cerebral swelling; general decompression and disappearance of ventricles and cistems; migration of median structure of brain no more than 0.5 cm; other complicated cranial injuries. Conclusion DAI can be diagnosed by combining clinical findings with CT signs.
