乙肝病毒感染对RF检测结果的影响及临床意义

目的研究类风湿因子(RheumatoidFactor,简称RF)在慢性乙型肝炎中的表达情况,并探讨其与慢性乙型肝炎误诊的关系及降低误诊发生的措施。方法检测217例乙肝患者血清中RF浓度,生化指标丙氨酸转移酶(ALT)、天冬氨酸转移酶(AST)和总胆红素(TBil),乙肝病毒标志物,即乙肝表面抗原(HBsAg),乙肝表面抗体(抗-HBs),乙肝e抗体(抗-HBe),乙肝核心抗体(抗-HBc),乙肝e抗原(HBeAg),及补体C3、C4。比较乙肝活动组与非活动组的RF阳性率,RF阳性组与RF阴性组间各乙肝检测项目的阳性率,对RF与各生化、补体检测项目间进行关联分析。结果乙肝活动组RF阳性率高于非活动组(P<0.05);RF阳性组的HBsAg阳性率高于阴性组(P<0.05),RF阴性组的抗-HBs和抗-HBe阳性率均高于阳性组(P<0.05);RF与ALT、AST、TBil间存在正相关(P<0.05)。结论慢性乙型肝炎患者可能因乙肝病毒感染后机体免疫紊乱,致使RF检测阳性,临床上结合病史、临床表现、常规生化及其他辅助检查加以分析,可减少风湿病的误诊率。

The function and clinical significance of hepatitis B virus infection on RF test results
