传统器械经脐入路单孔腹腔镜胆囊切除术40例分析

目的探讨传统器械经脐入路单孔腹腔镜胆囊切除术(LC)的安全性、可行性及技术要点。方法回顾性分析我院2010年5月～2011年8月使用传统器械经脐入路单孔腹腔镜胆囊切除40例患者的临床资料;所有病例都经过B超或CT确诊为慢性结石性胆囊炎或胆囊息肉,手术指征包括:①非急性炎症期的胆囊炎或胆囊结石;②胆囊息肉;③非急性炎症期的胆囊良性病变。结果本组40例均获得手术成功,手术时间40 min～120 min,平均73.9±20.9 min。未置腹腔引流管,无出血及胆漏、胆道损伤等重大并发症,平均住院时间为4天。术后随访1个月～3个月,腹部未见明显手术疤痕。结论使用传统器械经脐入路单孔腹腔镜胆囊切除术是安全可行的,美容效果极佳,但手术操作难度较大,初学者需要经过一定量的练习方能熟练掌握。

Transumbilical single-port laparoscopic cholecystectomy by traditional instrument:a study of 40 patients

Objective To study security,feasibility and technical point of transumbilical single-port laparoscopic cholecystectomy by traditional instrument.Methods The data of 40 patients with transumbilical single-port laparoscopic cholecystectomy by traditional instrument in our department from May 2010 to August 2011 were analyzed retrospectively.The diagnosis were made by ultrasound and CT.Surgical indications include:① Non-acute inflammation time cholecystitus or cholecystolithiasis;② Gallbladder polyp;③ Non-acute inflammation time gallbladder benign disease.Results All the operations were successfully completed.Operative time was from 40 to 120 min,Average 73.9±20.9 min.No setting at the abdominal cavity drainage tube,no surgical complications such as hemorrhage,cholerrhagia and Biliary duct damage.The average in hospital time is 4 days.Postoperatively from 1 to 3 months following up adominal wall scar was not obvious.Conclusions Transumbilical single-port laparoscopic cholecystectomy by traditional instrument is technically safe and feasible.But it′s more difficult to manipulate.
