中药热熨联合穴位按摩治疗中风患者偏瘫肢体肿胀的疗效观察

目的 探讨中药热熨联合穴位按摩在中风偏瘫肢体肿胀患者中的应用效果。 方法 选取我院2016年6月-2018年10月收治的100例中风偏瘫肢体肿胀患者为研究对象,采用随机数学表法将其分为实验组和对照组,每组50例。对照组给予常规治疗和护理,实验组在此基础上给予五子散中药热熨联合穴位按摩。对比2组治疗效果,患肢足踝和小腿周径、简易上肢功能检查量表(simple test for evaluating hand function,STEF)评分、运动功能评定量表 (function assessment scale, FMA) 以及日常生活活动量表 (Barthel指数)评分、数字评分疼痛量表 (numerical rating scale, NRS) 评分。 结果 干预后,实验组治疗效果优于对照组(χ²=4.891,P=0.027) ;FMA评分、Barthel指数以及NRS评分较对照组显著改善 (t=8.731,P<0.001; t=10.102,P<0.001; t=5.329,P=0.001);2组STEF评分、足踝及小腿周径在时间、组间、交互效应上比较,差异均有统计学意义 。 结论 中药热熨联合穴位按摩能有效减轻中风患者偏瘫肢体肿胀,促进肢体的功能康复,提高患者生活质量。

The curative effect of the ironing with Chinese medicine combined with acupoint massage on the limb swelling of apoplectic hemiplegia

Objective To investigate the effect of Chinese herb hot compress combined with acupoint massage on the limb swelling of stroke survivors. Methods Totally 100 patients with apoplectic hemiplegia and limb swelling were selected and randomly divided into an experimental group and a control group, each of 50. Both groups were given routine medications and nursing, while the experimental group was additionally provided with Chinese herb Wuzisan hot compress combined with the acupoint massage. Before and after the treatment, both groups were evaluated using the simple test for evaluating hand function (STEF), the motor function assessment scale (FMA), activities of daily living scale (Barthel index) and numerical rating scale (NRS) before and after the intervention. The cross-section diameter of the affected ankles and legs were also measured. Results After the treatment, the treatment effect of the experimental group was significantly better than that of the control group (χ²=4.891, P=0.027), as well as the FMA and Barthel index and NRS pain scores of the experimental group improved more significantly than that of the control group(t=8.731, P<0.001; t=10.102, P<0.001; t=5.329, P=0.001). There were statistical differences in the time, inter group, and interaction effects of the STEF score and ankle and calf circumferences between 2 groups. Conclusion The Chinese herb hot compress combined with the acupoint massage can significantly relieve the limb swelling, improve the function rehabilitation and promote the life quality of the patients with hemiplegia after stroke.
