胎儿脐带绕颈的4251例彩色多普勒超声诊断

目的应用彩色多普勒超声诊断脐带绕颈,从而探讨彩色多普勒超声诊断胎儿脐带绕颈的准确率,分析误诊原因,对其临床诊断、治疗及分娩方式提供依据。方法应用彩色多普勒对18 269例孕妇产前超声检查检出脐带绕颈与胎儿分娩后结果进行对照分析。结果本院12 869名孕妇产前检查,分娩后脐带绕颈者4 530例,占分娩总数的24.80%,超声诊断脐带绕颈4 251例,诊断准确率93.84%。结论彩色多普勒超声对胎儿脐带绕颈诊断安全、可靠,准确率高,具有相当重要的价值。为临床选择分娩方式及降低围产儿病死率提供了可靠的依据,具有积极的指导意义。

Diagnosis of 4 521 cases of fetal umbilical cord around neck by color Doppler ultrasonography

Objective To discuss the accuracy of diagnosing fetal cord around neck by color Doppler ultrasonography,to analyze the causes for misdiagnosis,and to provide the basis for clinical diagnosis,treatment and delivery mode by using color Doppler ultrasonography in diagnosis of umbilical cord around neck.Methods Cases of fetal cord around neck were diagnosed in 18 269 pregnant women by prenatal ultrasound detection,and their diagnosis was compared with the result after childbirth.Results In 12 869 pregnant women of prenatal examinations in this hospital,4 530 cases of cord around neck were found after childbirth,accounted for 24.80% of total deliveries,and 4 251 cases of cord around neck had been found by prenatal ultrasound diagnosis,the diagnostic accuracy was 93.84%.Conclusion Color Doppler ultrasonography is safe,reliable and accurate and it has very important value for diagnosis of fetal cord around neck,and it has a positive significance for providing reliable basis for choosing clinical delivery mode and reducing the perinatal mortality rate.
