BiPAP呼吸机和硝普钠联合应用治疗高血压急性左心衰临床观察

目的:探讨在硝普钠治疗基础上无创双水平气道正压通气(BiPAP呼吸机)抢救高血压突发急性左心衰的临床效果。方法:将52例在急诊及内科住院期间发生急性左心衰的高血压患者分成两组,对照组30例予硝普钠治疗,治疗组22例在此基础上加用BiPAP呼吸机正压通气治疗,记录两组患者治疗前后心率、脉搏、呼吸、血压、血氧饱和度(SPO2)及临床表现变化情况。结果:治疗组主要临床症状及体征消失或改善快,治疗后血压、心率、呼吸和手指血氧饱和度(SaO2)均有显著改善(P<0.01)。结论:BiPAP呼吸机和硝普钠联合应用治疗高血压急性左心衰疗效肯定。

The Efficacy of Non-invasive Bilevel Positive Airway Ventilation (BiPAP) with Nitroprusside in Patients with Acute Left Heart Failure associated with Hypertension

Objective:To investigate the efficacy of non,invasive bilevel positive airway ventilation (BiPAP) in patients with acute left heart failure associated with hypertension. Methods: 52 patients with acute left heart failure associated with hypertension were enrolled. All patients received the Nitroprusside to control the blood pressure and heart failure. Meanwhile, patients were randomly allocated to treatment group (n=30) and control group (n=22). Besides Nitroprusside, patients in treatment group received BiPAP as well. The clinical manifestation, including heart rate,pulse, respiration rate, blood pressure and SPO2 were recorded before and after the treatment. Results: Patients in treatment group improved more quickly and thoroughly than control group (P < 0.01). Conclusions: BiPAP combined with Nitroprusside is efficient for acute left heart failure associated with hypertension.
