降钙素原检测对于耐药结核病的预测价值

目的观察血清降钙素原（PCT）在抗结核治疗时的动态变化,及早发现耐药患者。方法选择300例初治接受正规抗结核药物治疗的涂阳培阳肺结核患者,治疗前均做罗氏比例法药敏试验和血清PCT检测,并于抗结核治疗期间每个月做血清PCT检测,分析结核分枝杆菌耐药和敏感患者PCT动态变化规律,以便及早发现耐药患者。结果耐药组和敏感组肺结核患者治疗前血清PCT差异无统计学意义（P〉0.05）,治疗后1个月、2个月时2组血清PCT均降低,但组间差异无统计学意义（P〉0.05）,治疗3、4和6个月时耐药组无下降,与敏感组相比差异显著（P〈0.01）。结论血清PCT可作为肺结核患者抗结核治疗疗效评估指标,并对耐药结核有预测作用。

predictive value of procalcitonin detection for drug resistant tuberculosis

Objective To observe the dynamic changes of serum procalcitonin （PCT） in anti tuberculosis treatment, in order to find resistant patients early. Methods 300 cases initial treatment pulmonary tuberculosis patients with smear and culture positive accepting regular anti tuberculosis drug combination were chosed. LowenStein-Jensen （L-J） culture proportion method of drug sensitivity test and serum PCT were detected before treatment. And during the period of anti tuberculosis treat- merit,serum PCT was detected every month. The dynamic changes of PCT in mycobacterium tuberculosis drug resistance and sensitive patients were analyzed, in order to early detect resistant patients. Results The serum PCT had no significant difference between the resistant group and sensitive group of pulmonary tuberculosis patients before treatment （ P 〉 O. 05 ）. After treatment for 1 months and 2 months, the serum PCT of both groups decreased, but there was no significant difference between the 2 groups （P 〉 0.05 ）. After treatment for 3,4 and 6 months, the resistance group no longer decreased, and there was signifi- cant difference compared with sensitive group （P 〈 0.01 ）. Conclusion The serum PCT can be used as the curative effect evaluation index of anti tuberculosis treatment in patients with pulmonary tuberculosis, and has prediction effect on drug resistant tuberculosis.
