后路内固定联合前路病灶清除及椎间植骨治疗原发性腰椎间盘炎

目的：探讨一期后路内固定联合前路病灶清除及椎间植骨治疗原发性腰椎间盘炎的临床效果。方法：16例原发性腰椎间盘炎患者均行后路椎弓根内固定、前路病灶清除及椎体间植骨手术治疗。结果：16例患者术后即感腰腿痛明显缓解．床上能自行翻身；16例患者获得长期随访，随访4～20个月．所有病变间隙于3～6个月都达到骨性融合，未再出现腰痛及神经症状。结论；该手术能彻底地清除病灶、重建脊柱的稳定、有效地缓解腰腿痛，是治疗腰椎间盘炎的一个较好的方法。

Posterior Transpedicular Screw Internal Fixation and Anterior Debridement and Interbody Fusion for Treating Spontaneous Lumbar Discitis

Objective:To evaluate the clinical effectiveness of posterior transpedicular screw system internal fixation and anterior debridement and interbody fusion in spontaneous lumbar discitis.Methods: Twenty-three cases of spontaneous lumbar discitis were treated in the First and Second Affiliated Hospital of Nanchang university from September 2002 to february 2006.And posterior transpedicular screw system internal fixation and anterior debridement and interbody fusion were used in 18 patients.Results:All the patients suffering from lower back pain was obviously relieved after operation and could turn over by themselves.Then 16 cases were involved in a longitudinal study with follow-up for 4~20 months.The roentgenography showed the intervertebral fusion in 3~6 months after operation.No lumbodynia or neurologic complication occurred.Conclusion:The operation has advantages of thorough debridement,reconstruction of spinal stable, and quick relief of lumbodynia,which is a very effective and safe option for treatment of spontaneous lumbar discitis.
