摘    要:目的:探讨应用外科治疗风心瓣膜病合并慢性心房纤颤的治疗方法。方法:采用了孙衍庆教授设计的窦性心律重建手术,为10例风湿性心脏病和1例先心病肺动脉高压患者施行了手术,通过心房区域化减少心房损伤的范围;采用电热凝加缝合法以消除或减少并发症;同时通过对左房底部操作消除局部纤颤。结果:本组无住院死亡,术后均恢复为窦性心律,无心房麻痹发生,无心房出血并发症。结论:窦性心律失建手术对于治疗风心瓣膜病合并心房纤颤具有疗效满意、手术操作简化、并发症少的优点,是适合于在基层医院开展相对安全的手术方法。

Abstract:Objective: To study surgical treatment for chronic atrial fibrillation. Methods: Rhythm restoration technic was designed by Prof. Sun Yanqing. Minimizing the area of surgical incisions on the atrial wall; using electro thermal ablation as a substitute for surgical incision to reduce the complications; applying an additional surgical procedure on bottom of the left atrium to eradicate the regional fibrillations. Results: This sinus rhythm restoration technique was applied on 10 cases of rheumatic heart disease and one case of congential cardiovascular disease of primary pulmonary hypertension. There was no hospital fatality, no postoperative bleeding or A V blook demonstrated. Sinus rhythm was resuned immediately after surgery in all cases. Conclusion: The clinical results demonstrate that sinus rhythm restoration procedure is safe and effective in the treatment of chronic atrial fibrillation with minimal surigcal complications, especially, the operation procedure can be finished in basic hospital with simple equipments.
