EGF、VEGF在老年人胃溃疡中的表达及其与溃疡愈合的关系

目的研究EGF、VEGF在老年人胃溃疡中的表达及其与溃疡愈合的关系,以指导临床正确选用药物,提高老年人胃溃疡的愈合质量和治愈率。方法选择同期老年和中青年活动期胃溃疡各30例,抽取胃液和活检胃黏膜组织用ELISA法和免疫组化检测EGF、VEGF表达;两组均予常规剂量服用埃索美拉唑镁肠溶片4周后复查胃镜,了解溃疡愈合情况。统计分析各组数据,探索老年人胃溃疡愈合与EGF及VEGF之间的关系。结果老年组胃液中EGF表达明显低于中青年组,差异有统计学意义(P<0.001)。治疗后,老年组溃疡愈合率明显低于中青年组(P<0.001),老年组胃液中EGF和黏膜组织中EGF、VEGF的表达均明显低于中青年组(P<0.001)。结论老年人胃黏膜在溃疡愈合过程中EGF、VEGF低水平表达,可能是老年人胃溃疡愈合质量差和难治愈的主要因素之一,故在治疗老年人胃溃疡时用PPI抑酸药联合促胃黏膜EGF、VEGF表达的胃黏膜保护剂,将提高老年人胃溃疡的愈合质量,解决难治愈的问题。

Expression and Healing Relationship of EGF,VEGF in Elderly Gastric Ulcer

Objective To explore the expression of EGF and VEGF in gastric ulcer in the elderly and its relationship with ulcer healing,to guide clinical correctly select drugs,and to improve the quality of gastric ulcer healing and cure rate of the elderly.Methods 30 cases of elderly and 30 cases of young active gastric ulcer patients were selected.The extracted gastric juice and gastric mucosa organization from biopsy were examined by ELISA and immunohistochemistry for EGF and VEGF expression.The two groups were taken the conventional dosage of Esomeprazole for 4 weeks and then examined gastroscope for observing the ulcer healing.The relationship between the EGF,VEGF and the elderly gastric ulcer healing was explored.Results The expression of gastric juice EGF in the elderly group was significantly lower than that in the young group,the difference was statistically significant(P<0.001).After treatment,ulcer healing rate of the elderly group was significantly lower than that in the young group(P<0.001).The expression of EGF in gastric juice and EGF,VEGF in organization in the elderly group was significantly lower than that in the young group(P<0.001).Conclusion EGF and VEGF expression level of gastric ulcer healing in the elderly was low.This may be one of the main factors on poor quality of gastric ulcer healing and the elderly difficult to cure.It was used in the treatment of elderly gastric ulcer with PPI associating the drugs of promoting gastric mucosal EGF and VEGF expression,which can increase the quality of ulcer healing and solve the problem difficult to cure.
