阿奇霉素联合布地奈德雾化吸入治疗婴幼儿毛细支气管炎对照研究

摘    要:
目的 探讨阿奇霉素静脉滴注联合布地奈德混悬剂雾化吸入治疗婴幼儿肺炎支原体感染所致毛细支气管炎的疗效和安全性.方法 将96例毛细支气管炎患儿随机分为3组,每组32例.3组在常规给予氨茶碱和痰热清静脉滴注的基础上,A组静脉滴注阿奇霉素,B组静脉滴注阿奇霉素联合α-糜蛋白酶、地塞米松和利巴韦林加入生理盐水中雾化治疗,C组静脉滴注阿奇霉素联合布地奈德混悬剂加入生理盐水中雾化治疗.观察3组患儿治疗效果.结果 3组痊愈、好转及无效率比较差异有极显著性(χ2=20.25,P<0.01),总有效率比较差异有极显著性(χ2=20.10,P<0.01);A组与B组、C组总有效率比较差异均有极显著性(P<0.01),B组与C组总有效率比较差异无显著性(P>0.05);A组与B组临床症状消失时间及治疗时间均显著长于C组(P<0.01),A组气喘和肺部哮鸣音消失时间显著长于B组(P<0.01).结论 阿奇霉素静脉滴注联合布地奈德混悬剂雾化治疗婴幼儿毛细支气管炎起效快,疗效显著,安全性高.

Abstract:
Objective To explore the efficacy and safety of azithromycin iv gtt plus budesonide aerosol inhalation in the treatment of infant bronchiolitis causedby mycoplasma pneumomiae (MP) infection. Methods Ninety-six bronchiolitis infants were randomly assigned to three groups of 32 ones each. On the basis of routine aminophylline and Tanreqing injection iv gtt treatment for three groups, group A received azithromycin iv gtt, B did azithromycin iv gtt plus a-chymo- trypsin, dexamethasone and ribavirin aerosol dissolved in physiological saline and C did azithromycin iv gtt plus budesonide suspension aerosol dissolved in physiological saline. Treatment effectiveness of three groups was observed. Results There were very significant differences in recovery, improvement and ineffective rates among three groups (X^2=20. 25,P〈0.01) and significant differences in total effective rates (X^2 =20.10,P〈0.01) ; there were very significant differences in total effective rates between group A and Bor C (P〈0.01) and no between group B and C (P〈0.05); extinction and treatment time of clinical symptoms were significantly longer in both group A and B than C (P〈0.01), extinction time of asthma and lung wheezing were significantly longer in group A than B (P〈0.01). Conclusion Azithromycin iv gtt plus budesonide suspension aerosol takes effect more rapidly, has an evident effect and higher safety in the treatment of infant bronchiolitis.
