弥散性血管内凝血患儿凝血障碍的检测及其临床意义

摘    要:
目的探讨弥散性血管内凝血(DIC)患儿凝血障碍的检测及临床意义。方法用免疫法、发色底物法和凝固法分别测定39例DIC患儿血浆D二聚体(D-D)、抗凝血酶活性(AT:A)、凝血酶原时间(PT)、活化部分凝血活酶时间(aPTT)、凝血酶时间(TT)、纤维蛋白原含量(Fbg)等凝血及抗凝指标。结果DIC患儿的凝血指标与正常对照组儿童间差异有统计学意义,特别是D-D含量明显增高,AT:A明显减低,差异具有显著性意义(P〈0.01);DIC发生时死亡组AT:A阳性率达100%。结论各凝血指标特别是AT:A快速定量检测对DIC的诊断和预后评估有较高价值。

Abstract:
Objective Investigate the measurement of coagulation in children with disseminated intravascular coagulation (DIC) and its significance. Methods Plasm lever of D-Dimer ( D- D), antithrombin activity ( AT : A ), prothrombin time ( PT), activated partial thromboplastin time (aPTT), thrombin time (TT) and fibrinogen(Fbg) were tested by immunoological, color substrate and coagulation method. Results The levers of these parameters were significantly different between the DIC children and the normal children. Especially D-D increased and AT : A decreased significantly( P 〈 0.01 ). Sensitivities of AT : A are 100% in patient whom were died of DIC. Conclusion The fast quantitative tested of AT:A may be used for diagnosis in children with DIC and evaluation of the prognosis.
