滋水涵木法针刺对围绝经期女性干眼患者视觉相关生存质量的影响

目的评价“滋水涵木”法针刺治疗围绝经期女性干眼患者的疗效及其对视觉相关生存质量的影响。方法收集门诊就诊的围绝经期干眼患者40例，计算机随机法分为2组，分别给予人工泪液治疗（对照组）或针刺（主穴为太溪、太冲、三阴交、足三里、睛明、承泣、丝竹空透鱼腰）＋人工泪液治疗（治疗组）治疗，治疗前及治疗后2周分别观察患者的泪膜破裂时间（BUT）、基础泪液分泌量（SIT）。并对患者的主观症状评分及视觉相关生存质量评分（VRQOL）进行分析。结果试验组治疗后14d的SIT、BUT、主观症状评分、VRQOL总评分、VRQOL情绪状况评分等均优于治疗前（P〈0．05），而对照组仅治疗后14d的SIT及VRQOL评分优于治疗前（P〈0．05）；此外组间比较提示治疗14d后试验组SIT、VRQOL评分及VRQOL情绪状况评分的改善幅度优于对照组（P〈0．05）。结论泪液替代治疗和针刺治疗均可改善围绝经期干眼患者的症状和体征，与单纯泪液替代治疗比较，增加针刺治疗可进一步改善患者基础泪液分泌量及视觉相关生存质量，对患者情绪的改善作用亦较为明显，提示针刺配合泪液替代治疗是治疗更年期女性干眼的有效方法。

Effects of nourishing liver and kidney by acupttgcture for vision related quality menopausal women with dry eye

OBJECTIVE To evaluate the effects of ＂nourishing liver and kidney＂ by acupuncture for vision related quality of life in perimenopausal women with dry eye. METHODS Forty cases of perimenopausal women suffered from dry eye were collected and divided into 2 groups randomly by computer, which were treated by artificial tears therapy （control group） or acupuncture （the main point of the Taixi, Taichong, Sanyinjiao, Zusanli, Jingming, Chengqi, Sizhukong penetrating Yuyao） ＆ artificial tears therapy （treatment group） respectively. The tear break-up time （BUT）, Schirmer I test （SIT）, corneal fluorescein staining, the symptoms＇ score and vision related quality of life （VRQOL） scores of patients were detected and marked before treatment and 2 weeks after treatment. RESULTS SIT, BUT, subjective symptoms score, VRQOL score and VRQOL score of emotional status in the test group were better after 14d treatment than that before treatment （P〈0.05）, while only SIT and VRQOL score were better （P〈 0.05） in the control group. Moreover, the improvement of SIT, VRQOL score and VRQOL score of emotional status in the test group was better than the control group （P〈0.05） after 14 days of treatment. CONCLUSIONS Tear replacement therapy and acupuncture therapy both could improve symptoms and signs in perimenopausal patients with dry eye. Furthermore, additional improvement of SIT, VRQOL score and VRQOL score of emotional status could be found in patients when acupuncture therapy added to tear replacement therapy. This research suggested that tear re- placement therapy added to acupuncture was an effective method for dry eye treatment on perimenopausal women.
