社区高血压综合干预效果分析

目的分析社区高血压综合干预效果。方法根据《中国高血压防治指南》制定社区高血压病例随访干预方案,组建社区责任医生团队,对高血压病例进行分级管理,通过站点随访、上门访视等方式进行药物和非药物干预。结果 415例高血压病例,规范管理342例,规范化管理率为82.41%。干预前后高血压"三率"出现了显著变化,高血压知晓率由53.25%提高到95.90%,治疗率从干预前的22.89%上升到65.54%,血压控制率由30.84%上升至65.54%,高血压病例的高盐饮食和锻炼频率与基线调查时有差异,高血压社区综合干预效果显著。结论组建社区责任医生团队,实施社区高血压综合干预是控制高血压发病的有效措施。

The Effects of Comprehensive Intervention on Hypertension in Community

Objective To evaluate the effect of comprehensive intervention on hypertension in Caoe community. Methods Follow - up intervention program was carried out according to The Chinese hypertension prevention and cure guideline＇. The community physician team was established to guarantee the management of hypertension patients. Both drug and non - drug interventions were carried out by follow -up visit. Results 415 patients were included in the program, 342 of which were standardizedly managed （82. 41% ）. The awareness rate of hypertension among the participants increased from 53.25% to 95.90%, and 22. 89% to 65.54% for treatment rate , 30. 84% to 65.54% for control rate. Meanwhile, statistical significance was observed in both high salt diet rate and exercise rate compared to the baseline investigation. Conclusion Comprehensive intervention based on community physician team is an effective measure to control hypertension.
