非离子型造影剂对老年冠心病患者肾功能的影响

目的探讨老年冠心病患者接受非离子型造影剂后对肾功能的影响。方法选择老年冠心病患者64例,其中单纯冠心病22例作为对照组,合并高血压24例,合并糖尿病18例均作为观察组;再根据水化治疗与否分为亚组。全部患者接受造影剂前、后测定血BUN、SCr,计算CrCl并进行比较。结果造影后1～2 d,糖尿病组SCr升高,CrCl下降(P<0.05),5～7 d恢复并与造影前比较差异无统计学意义(P>0.05);未水化治疗的高血压组BUN增高(P<0.05),而糖尿病组BUN、SCr均增高(P<0.05)。结论非离子型造影剂对老年冠心病患者是相对安全的,但合并糖尿病患者更应预防造影剂肾病的发生。

Effects of nonionic contrast medium on kidney function of elderly patients with coronary heart disease

Objective To investigate effects of nonionic contrast medium on kidney function of elderly coronary heart disease patients.Methods 64 elderly patients with coronary heart disease were divided into three groups,22 with single coronary heart disease,24 with hypertension and coronary heart disease,and 18 with diabetes and coronary heart disease;they may be further devided into subgroups according to hydration regiments.BUN and SCr were measured and CrCl was calculated respectively before and after contrast medium injection in all patients,and values of before and after contrast medium injectigng were compared with each other.Results 1～2 days after contrast medium injection SCr increased and CrCl decreased(P<0.05) in the group with diabetes,then SCr and CrCI recovered to the basal level(P>0.05)after 5～7 days;BUN increased(P<0.05)in the hypertension group,while both BUN and SCr increased(P<0.05)in the diabetes group,both groups were not treated with hydration regiments.Conclusion Nonionic Contrast Medium is relative safe to elderly patients with coronary heart disease,but contrast inclued nephropathy should be prevented in those patients with diabetes.
