胫骨与软组织严重损伤的修复治疗

目的 探讨胫骨缺损软组织严重损作斩治疗原则和方法。方法 对38例胫骨缺损和骨不愈合患者，应用彻底清创技术，有效地抗生素，外固定器或支持钢板固定，皮瓣转移修复创面，隧道植骨治疗。结果 全部患者疗程为6－28个月，平均14个月达到骨性愈合，伤口创面愈合，感染得到控制，保全了肢体，并保证肢体长度均衡和功能的恢复。结论 采取综合规范治疗措施，清创、抗菌素及皮瓣转移在修复创面起积极作用，使用外固定器可稳定骨折保证肢体长度，带血管蒂腓骨移位隧道性植骨可保证植骨成活亦可避免已愈合瘢痕伤口再手术而不愈合的缺点。

Reparative treatment of tibial bone defect and serious injury of soft tissue

Objective To discuss the principles and the different methods of the treatment of bone defect and serious injury of soft tissue. Methods Thirty eight patients with bone defect and serious injury of soft tissue were retrospectively researched. Effective antibiotic administration and completely debridement, external fixators or internal fixation, transferable cutanous flap, tunnel bone graft were used. As soon as the wound was healed up, different type bones were grafted. Results The infection was completely controlled within 6 to 28 months (mean 14 months), The wound and bone went on healing, and the length of the limb was maintained. Conclusion Complex method of the treatment has excellent bone healing with satisfactory functional recovery.
