先天性甲状腺功能减退症对儿童肝功能的影响

目的观察先天性甲状腺功能减退症（CH，甲低）儿童的肝功能改变，及与血清甲状腺激素（TH）水平的相关性，探讨TH对儿童肝功能的影响。方法对40例CH儿童和35例健康儿童进行肝脏彩超及肝功能检查，同时用化学发光法测定血清游离三碘甲状腺原氨酸（FT3）、游离甲状腺素（TT4）、总三碘甲状腺原氨酸（TT3）、总甲状腺素（讯）和促甲状腺素（TSH）水平，对肝功能指标和血清TH水平进行相关性分析。结果与健康儿童组相比，CH组血清FT3、TT3、FT4、TT4水平显著下降，TSH水平显著升高（P〈0．001）；CH组肝功能异常明湿高于健康组（P〈0．001），且肝功能异常与FT3、FT4、TT3、TT4呈显著负相关，与TSH呈显著正相关（P〈0．05），两组肝脏彩超检查无显著差异（P〉0．05），表明先天性甲状腺功能减退症会对患儿的肝功能造成一定的损伤。结论CH对儿童肝功能产生显著影响，临床医师应加强对此类疾病的重视。

Effects of congenital hypothyroidism on liver function in children

Objective To observe the effect of congenital hypothyroidism （CH） changes in liver function of children, and serum thyroid hormone （TH） and the level of correlation, the effect of TH on hepatic function in children. Methods Liver ultrasound and liver function tests in 40 cases of children with CH and 35 healthy children, three si multaneous determination of serum free triiodothyronine by chemiluminescence（FT3）, free thyroxine （TY4）, total three triiodothyronine （TT3）, total thyroxine（TT4） and thyrotropin （TSH） level, the correlation analysis on the index of liver function and serum TH level. Results Compared with healthy children, the levels of serum FT3, TT3, CH, FT4, TT4 levels decreased significantly, TSH increased significantly （P 〈 0.001）; CH group of abnormal liver func tion were significantly higher than those in healthy group （P 〈 0.001）, and abnormal liver function and FT3, FT4, TT3, TT4 showed a significant negative correlation, a significant positive correlation with TSH （P 〈 0.05）, no significant differ- ences between the two groups of liver ultrasound examination （P 〉 0.05）. Conclusion CH has significant effect on chil- dren＇s liver function, clinicians should strengthen the importance of these diseases.
