聚合酶链反应DNA扩增对肺结核疗效监测

目的:探讨聚合酶链反应(PCR)痰结核杆菌DNA扩增对肺结核疗效监测的临床实用价值。方法:对31例初治涂阳肺结核患者进行2年的随访研究,定期行痰涂片抗酸染色和PCR检查。结果:31例中有27例痰PCR在治疗结束前转阴,另4例痰PCR持续阳性时间长达12月左右。31例中有3例初治有效后复发,2例为PCR持续阳性患者,1例为痰涂片和PCR复阳患者,PCR复阳时间比涂片早2月。485份痰标本痰PCR和涂片检查总符和率为88.9%(431/485)。结论:提示痰PCR是检测肺结核疗效敏感的方法,痰PCR和涂片检查总符合率较高,复发多见于痰PCR持续阳性患者。

Monitor of the curative effects of pulmonary tuberculosis by polymerase chain reaction

Objective:To evaluate the clinical application value of polymerase chain reaction(PCR) in monitoring therapeutic effect of lung tuberculosis.Methods: 31 patients with tuberculosis were included.They all had positive examination result in film preparation of sputum after the first effective therapy,and were followed up for two years through sputum PCR and film preparation examination.Results: 27 patients turned into negative from positive in sputum PCR examination before the end of treatment.The other 4 patients had positive result for 12 months.3 patients recurred,2 were persistently positive in sputum PCR examination,and 1 were both positive in PCR and film examination.The positive result of PCR appeared 2 month earlier than that of the film examination.The accordance ratio of PCR and film examination in 485 sputum specimens was 88.9%(431/485).Conclusion: Sputum PCR is sensitive in detecting the therapeutic effect of lung tuberculosis.It is highly consistent with the film preparation examination.The patients with persistent positive result of PCR examination are prone to recur.
