摘 要: | 目的总结慢性乙型肝炎(chronic hepatitis B,CHB)患者血清自身抗体特征和乙型肝炎病毒(HBV)基因型的分布情况,探讨HBV基因型与自身抗体的相互关系。方法对196例CHB患者进行HBV基因分型和血清自身抗体检测,对30例健康体检者进行自身抗体检测。分析HBV基因型、HBV-DNA载量、生化指标、年龄、性别与自身抗体的关系。结果 196例CHB患者中67例(34.2%)自身抗体阳性,阳性率明显高于正常对照组(P0.05);196例CHB患者中基因B型41例,C型134例,B+C型21例,其自身抗体阳性者分别为12例(29.3%)、46例(34.3%)、9例(42.9%),自身抗体阳性率在不同基因型之间差异无统计学意义;自身抗体阳性组和阴性组、高滴度组和低滴度组患者比较,其性别比例、年龄分布、HBV-DNA、生化指标的差异均无统计学意义。结论 HBV慢性感染可以诱导机体产生低滴度非器官特异性自身抗体,且不同HBV基因型诱发自身免疫反应的能力无明显差异。

The relationship between HBV genotypes and serum autoantibody in patients with chronic hepatitis B |

Abstract: | Objective To investigate the prevalence of autoantibodies in different genotypes of HBV, and to explore the relationship between HBV genotypes and serum autoantibodies in patients with chronic hepatitis B (CHB). Methods One hundred and ninety-six CHB patients were enrolled in the study. Serum autoantibodies and HBV genotypes were detected, the relationship between the autoantibodies and HBV genotypes, HBV-DNA level, biochemical markers, demographic features was analyzed. Results Autoantibodies were detected in 67/196 (34.2%) of CHB patients and 10.0% of matched normol controls (P < 0.05). Among 196 CHB patients, genotype B were 41, genotype C were 134, genotype B+C were 21. The prevalence of autoantibodies in genotype B, C, and B+C were 12 (29.3%), 46 (34.3%), and 9 (42.9%), respectively. There was no significant difference of HBV genotypes, HBV-DNA levels, biochemical markers, demographic features between autoantibody positive group and negative group. Conclusions HBV chronic infection can trigger autoimmune activation and elicit non-organ-specific autoantibodies (NOSAs) at low titer, and the prevalence of autoantibodies in genotype B, C, B+C is similar. |
